Care Coordinator

An exciting opportunity has arisen for a Care Coordinator with 3 years’ experience to join a well-established home care provider. This full-time role offers excellent benefits and a salary range of £24,000 - £25,000.

As a Care Coordinator, you will coordinate care services, ensuring the smooth operation of client rosters and providing assessments for new and existing clients.

You will be responsible for:

• Ensure care is provided in line with relevant policies and regulations.
• Conduct client assessments to determine individual care needs.
• Perform regular reassessments of current care plans.
• Liaise with families, medical professionals, and other stakeholders.
• Carry out spot checks, supervisions, and appraisals for care staff.
• Efficiently manage staff rotas, ensuring compliance with hours and breaks.
• Ensure staff recruitment files are up-to-date following interviews.

What we are looking for:

• Previously worked as a Care Coordinator, Care Supervisor, Care Worker, Support Worker, Care assistant or in a similar role.
• At least 3 years’ experience in providing care.
• Knowledge of the Bucks area.
• Valid driving licence.
• Right to work in the UK.
